County Psychiatric Hospital
Nagykallo

Our mission: The Szabolcs-Szatmar-Bereg County Administration’s Psychiatric Hospital is faithfully dedicated to help all people seeking or needing professional care because of their psychiatric, mental, or emotional problems.

Our past runs back over more than 100 years. Our present is characterized by innovation and eagerness to change. These foundations make our future plans, a little daring as they may seem, to be apparently realistic and feasible.

Our efforts focus on the future: therefore we continue to volunteer in promoting social destigmatization of all people with mental disorders, and we endeavour to establish a new, complex Psychiatric Methodological Centre to provide full service across all components of the prevention-treatment-care-rehabilitation spectrum. Our primary goal is to offer treatment, rehabilitation and reintegration to the largest possible proportion of people struggling with mental health problems.

The Past

Once Nagykálló was the centre of Szabolcs County. The building of the County Hall where the Psychiatric Hospital is today was designed by the Italian Aprilis brothers in late Baroque style in the middle of the 1700s. Once county balls were organized in the building but later it became the local prison. In 1876 Nyíregyháza became the centre of Szabolcs county and the County Hall building was not used. After it became empty it was offered to be the home of the lunatic asylum. First it was a part of the Budapest Lipótmező and was called the Hungarian Royal State Madhouse and it had 340 beds. It became independent in 1926 and it had patients not only from Szabolcs County but from Szatmár, Bereg, Ung and Hajdú counties as well.
In 1992 co-educational departments and special out-patient departments were organized. In 1996 the County Government changed its name to Psychiatric Hospital and its new name expresses that in addition to the medical treatment of mental patients it is designed to provide excellent care in areas of therapy and rehabilitation.

The Present

The development of provision of psychiatric services is very important in Hungary because continuous stress caused by unemployment is a serious problem not only in underdeveloped regions but everywhere in Hungary. Another problem for the provision of psychiatric services is the lack of professionals throughout the country. One possibility of reducing the lack of qualified professionals is the expansion of provision to other regions too. Another national problem is that the provision of psychiatric services is not as up to date as in other countries and the number of addicted individuals is increasing gradually every year.

Quality is one of the key words in the medical treatment of the patients. The presence of the professionals and the work environment is very important in the quality assurance system of our hospital.
Today each hospital should have a quality assurance system that is required by the International Standards. One of the basic ideas of the system is “Say what you do! Do what you have said!”
There are annual revisions and audits in every 3rd year. Thus, continuous quality development is an important part of the hospital’s everyday work.

There are 423 staff personnel in the hospital and the staff consists of doctors, nurses and other paramedical staff. The hospital supports continuing education. One of the main emphases of the hospital is that all staff should be service-oriented and patients should be treated as partners.

In the medical treatment the role of surroundings/environment is very important. Reconstruction in the hospital started in 2006. By 2008 the Out-Patient Centre and the In-Patient Department will have been up-dated and our hospital will be able to provide hotel like provision of services.

Present services:
Provision of services to “classical” psychiatric patients
-acute psychiatric services
-chronic psychiatric services
-rehabilitation services
-addiction services

Services for other psychiatric problems:
-panic disease
-stress management
-new addiction services to include: mobile phone addiction, internet addiction and shopping addiction etc.

We consider our patients as partners and we treat them not only with medicines but we use multiple approaches for treatment alternatives that include cultural, social and other modalities that improve the success of potential cure.
We approach psychiatric problems as complex and that they require not only medical treatment but a multi-focused approach for the addicted as well as chronic and acute psychiatric problems. It is equally important for us to provide prevention services as well.

The first step for our patients is medical treatment. From here they go into a rehabilitation program that helps them to get back to the everyday life. The goal of rehabilitation is to make our patients able to adapt to society and to make society open to accepting them.
In case of dementia, geronto-psychiatric and schizoid patients we develop services that are aimed the provision of continuing services to these types of chronic conditions.

Addiction

In Hungary the number of people suffering from different addictions is very high. About one million people are alcohol dependent, the number of smokers is increasing together with the number of those who use sedatives and sleeping pills and there are more and more drug users.
The hospital has an out-patient centre in Nyíregyháza where professionals – psychiatrists, psychologists, social workers, lawyers - try to deal with all types of addiction. We are currently working with patients that have addictions to alcohol, prescription medicine, gambling, shopping, the Internet, mobile phones and others. The presence of different professionals is very important because treatment of addicted patients can be successful only if there is a well-qualified team. Patients need help not only with their somatic and psychic complications but require help with severe existential and moral problems, problems in their relationships and because of using illegal drugs they may have legal problems that require assistance. We use medical therapy together with psychotherapy as well as assistance in their social life.

Our hospital is very proud of being the centre of the Hungarian Neuro-psychopharmacological Association (HCNP). The goal of the Association is to help the development of the neuro-psychopharmacology, to provide education, further education for professional staff and to cooperate with Hungarian and international associations.

The Future

According to the WHO’s 2006 survey by 2020 most of the population (4 out of 10) will have had permanent inability to work because of psychiatric problems. That’s why it is necessary to found a Psychiatric Methodological Centre.
Using the Yale University’s Rehabilitation Ladder System the goal would be the decrease and potential cessation of psychiatric symptoms and to create conditions for re-integration into the everyday life.
In the centre we would like to create effective psychotherapeutic rehabilitation areas where those patients who complete medical treatment can try to adapt and assume a normal life.
Many psychiatric patients suffer from locomotor disorders. We would like to create a physiotherapeutic centre for such patients.
For addicted and other patients we would like to create a patient-occupational program with different activity therapies.
Stress management will be another important field in our therapeutic program.

We would like to create a new model in Hungary. One of our goals is to decrease the number of the pensioned off population and to create training courses for them.
We would like to be a teaching hospital that would serve as practice field for future professionals. In the interest of the high-standard health care we place great importance on and support scientific research.

Our other goal is to provide for patients not only from our county but to be the centre of the whole northern Hungarian region, to have new contacts with other psychiatric institutions and with the society. We are building contacts with both Ukrainian and Romanian universities.

Last year we started the “Open Doors for Health” program.. We try to organize programs that call the society’s attention to the fact that they have to accept people with psychiatric disorders.
On the World Day of the Ill People we tried to give help to people with decreased ability to work and in the Burn-out Conference we tried to call our colleagues attention to the danger of burning out.

Our Help of Helpers Program wants to make life easier for people who work in health settings. It is very important that those who work in health care should work in a good environment and that they have and maintain a good physical and mental condition. Good salary and other benefits help the staff to maintain their own health which is important to assisting patients.

MENTA is the magazine of the hospital. It deals with health protection and has articles about high-standard health care, professional news but it introduces the everyday life and goals of the hospital. There are a lot of people who live without help all over the country and we are trying to open our gates and to help them to enter it without feeling ashamed.
